Register with our practice

If you wish to register as a new patient please ask at reception for a registration form and health questionnaire or select the link below to be taken to the online registration page.

You will be registered with the practice rather than an individual GP but you are entitled to express a preference. The practice will do its best to let you see the doctor of your choice, but it might not always be possible.

Register Your New Baby

If you are the proud parent of a new baby, you may not be thinking about the need to register him/her with a doctor, but it is in the baby’s best interest to do this as soon as you can. There are forms at the surgery that you can use to register your baby.

When the surgery is informed of your baby’s birth, we will send Mum a six week post natal appointment and your baby’s first immunisation appointment. If we do not have a registration for your baby we will include the appropriate form.

Catchment Area (Practice Boundary)

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.

Named GP

From 1st April 2015, GP Practices are required to allocate every patient with a named GP. This does not affect your treatment or care at the Practice and you can continue to see any GP of your choice as normal.

If you are unsure of whom your named doctor is please ask a receptionists.

Accessing someone else's information

Accessing someone else’s information

You may find it helpful if someone else, usually a family member, close friend or carer, has online access to your GP record. They can book appointments for you, order repeat prescriptions, or even to access your detailed care record on your behalf to help with your care. This is called proxy access.

The practice can consider proxy access to the GP record we hold here for any patient who is, in the opinion of their usual GP, freely able to give their fully-informed consent for their nominated proxy.

If you are the patient and you wish to do this, you can pick up a proxy access form from the surgery at the front desk. Your GP may wish to discuss the matter with you before consenting.
